Stalled and Failed flags

Stalled and Failed are flags on a card in Doing. They are not columns.

Stalled

A card becomes stalled when its agent has gone quiet beyond the expected heartbeat window.

Failed

A card is marked failed after an error event. The card stays visible in Doing so a person can inspect and recover it.
